Incident Tracking: Incident Details

You've chosen an existing incident or created a new one and have clicked the Incident Details tab, which allows you to display and modify of Incident details. 

  • The Incident ID is displayed and is not editable. 
  • The Incident Date, Time and Location previously entered are displayed. These fields can be modified. 
  • The Description of the incident can be modified in the field Description with a maximum of 4000 characters.
  • Data entered into the Incident Tracking module is used within Trillium as well as reported to OnSIS.
  • In the annual submission to OnSIS the Incident Location, Infraction Type, Authority Type, Outcome, Program Type and Status are reported.
  • In the C.S.I. module of Trillium the Incidents the student has been a participant in are detailed.
  • The only date format accepted by the application is DD/MM/YYYY.
  • The only time format accepted by the application is the 24 hr. format, aka. Military Time.

Additional Incident Information

Police Information

Select the Police Contacted box if the Police were contacted regarding the Incident.

  • Enter the date the Police were contacted.
  • Enter the Police Officer's name the Incident was reported to, or who responded to your scene.
  • Enter the Police Officer's Badge Number, the Station ID, Report Number, and the Police Station's Phone Number the Incident was reported to. 

Fire Department

Select the Fire Contacted box if the Fire Department was contacted regarding the Incident.

  • Enter the date of the Fire Department was contacted. 
  • Enter the Fire Department Captain's name the Incident was reported to, or who was in charge of your scene.
  • Enter the Fire Station ID and Fire Department's Phone Number the Incident was reported to.

Ambulance

Select the Ambulance Contacted box if an Ambulance was called regarding the Incident.

  • Enter the date of the Ambulance was contacted. 
  • Enter the Ambulance ID and the Ambulance Department's Phone Number the Incident was reported to.

Community Agency

Select the Community Agency Contacted box if a Community Agency was contacted regarding the Incident.

  • Enter the date the Community Agency was contacted. 
  • Enter the name of the Agency; the Staff member the Incident was reported to, or the Staff member who has been assigned to the Incident, and the Community Agency's Phone Number the Incident was reported to.

Staff Information

Select the name of the school Staff member from the picklist the Incident was reported to. The Staff Type is a read-only field of the Staff member you chose.
