How do I know if my form submission was successful

What happens if you have completed a response in Permission Click?

When you receive an invitation to fill out a form, you’ll be able to see at the top of the form the steps required to complete your submission successfully.

Some forms may have more or fewer steps than others.

Before you can submit your response, you must check the “I Agree” box.

Once you Agree and click the Submit button you will see the “Your response has been successfully recorded!” page.

Additionally, you will receive a PDF confirmation in your email with the subject line “Permission Click - Response Recorded.”
